Managing Anxiety, Substance Dependence & Emotional Dysregulation
This case study outlines the therapeutic support provided to a 29-year-old male presenting with persistent high anxiety, substance misuse, emotional instability, and elevated neurotic traits impacting daily functioning, relationships, and overall wellbeing. The individual experienced ongoing patterns of excessive worry, hypervigilance, overthinking, emotional reactivity, and difficulty managing stress, alongside reliance on substances as a means of coping with internal distress and interpersonal challenges.
At intake, the client presented with heightened anxiety symptoms, poor emotional regulation, low confidence, disrupted routines, social withdrawal, and difficulties maintaining healthy relationships and consistency in employment and personal goals. Substance use had progressively become intertwined with avoidance behaviours, emotional suppression, and maladaptive coping patterns, contributing to further psychological distress and reduced functioning across multiple life domains.
A detailed intake assessment and biopsychosocial evaluation were completed to explore mental health concerns, substance use patterns, behavioural triggers, interpersonal functioning, emotional processing, and protective factors. From this, a collaborative therapeutic support plan was developed with a focus on anxiety management, emotional awareness, behavioural insight, relapse prevention, and the development of sustainable coping mechanisms.
The intervention adopted a person-centred, trauma-informed, and strengths-based framework, incorporating one-on-one counselling, psychoeducation, grounding and mindfulness strategies, emotional regulation techniques, and reflective therapeutic interventions designed to improve self-awareness and resilience. Sessions also focused on identifying negative thinking patterns, reducing emotional impulsivity, strengthening communication skills, and building healthier routines and support networks.
Throughout the intervention period, consistent engagement and collaborative care contributed to meaningful progress across several areas of functioning. The client demonstrated increased participation in therapeutic sessions, improved emotional insight, healthier responses to stress and conflict, greater openness in communication, and movement toward personal, social, and vocational goals, including engagement in casual employment and more stable day-to-day functioning.

02. Care Approach
An initial therapeutic engagement process was undertaken to establish safety, rapport, and a clear understanding of the client’s presenting concerns. Early sessions focused on identifying patterns relating to anxiety, emotional overwhelm, avoidance behaviours, substance use, and interpersonal difficulties impacting the client’s overall functioning and wellbeing.
Pre- Assessment
Foundational sessions explored the client’s emotional history, coping patterns, lifestyle stressors, and behavioural responses to anxiety-provoking situations. Clinical observations and therapeutic discussions assisted in identifying areas requiring immediate support, while also establishing realistic and achievable goals for ongoing intervention.
Trigger Awareness
Counselling sessions focused on increasing the client’s understanding of emotional triggers, stress responses, and the relationship between thoughts, behaviours, and substance use. The client was supported to recognise patterns contributing to emotional escalation, social withdrawal, and self-sabotaging behaviours, while developing greater insight into healthier responses.
Personalized Therapy
Therapeutic interventions were tailored to the client’s presentation, incorporating person-centred counselling, psychoeducation, grounding strategies, mindfulness techniques, and strengths-based approaches. 04. The Outcomes

Over time, increased consistency in therapeutic participation contributed to improved emotional regulation, greater confidence in communication, and healthier responses to stress and interpersonal conflict. The client began demonstrating stronger self-awareness, improved motivation toward personal goals, and a greater willingness to implement practical coping strategies outside of sessions. Progress was also observed in the client’s ability to reflect on emotions, manage challenging situations more effectively, and engage more positively in social and vocational environments.
